Privacy Statement Practice Optimal Health Lab

OPTIMAAL GEZOND LAB / is responsible for the processing of personal data as described in this privacy statement. Personal data is processed carefully, in accordance with the requirements of the General Data Protection Regulation (GDPR) which came into effect on May 25, 2018.

For proper treatment, it is necessary for me, as your treating therapist, to keep a file. This is also a legal obligation imposed by the Medical Treatment Agreement Act (WGBO/Wkkgz). Your file contains notes about your health condition and data about the examinations and treatments performed.

The file also contains data necessary for your treatment that I have requested from another healthcare provider, for example your general practitioner, with your explicit consent.

I do my best to safeguard your privacy. This means, among other things, that I:

  • Handle your personal and medical data carefully,
  • Ensure that unauthorized persons do not have access to your data.

As your treating therapist, I am the only one with access to the data in your file.

I have a legal duty of confidentiality (medical professional secrecy).

The data from your file may also be used for the following purposes:

  • To inform other healthcare providers, for example, if the therapy has been completed or when referring to another practitioner. This only happens with your explicit (written) consent.
  • For use by a substitute during my absence.
  • For anonymized use during peer review.
  • A small part of the data from your file is used for financial administration, so that I, or my administrator, can prepare an invoice.

If I want to use your data for any other reason, I will first inform you and explicitly ask for your consent.

You have the right to inspect your client file at any time.
You have the right to have your data deleted at any time.
The data in your client file will be kept for 20 years, as legally required (WGBO).

PRIVACY ON THE HEALTHCARE INVOICE

The healthcare invoice you receive contains the data required by the health insurer, so you can claim this invoice from your health insurer.

  • Your name, address, and place of residence.
  • Your customer number and/or your date of birth.
  • Insurance and/or policy number.
  • The date of the treatment.
  • A brief description of the treatment.
  • Performance code.
  • The cost of the consultation.

COLLABORATION WITH OTHER ORGANIZATIONS

From my practice, I collaborate with the following organizations regarding the processing of your data, and I have entered into a processing agreement with them:

  • MijnDIAd digital patient/client administration.
  • Google.
  • Facebook.
  • Accountant / Bookkeeper.
  • Shopify / Web administrator

Access, adjust or delete data
You have the right to access, correct or delete your personal data. In addition, you have the right to withdraw any consent for data processing or to object to the processing of your personal data by OPTIMAAL GEZOND LAB, and you have the right to data portability. This means that you can submit a request to us to send the personal data we hold about you in a computer file to you or to another organization specified by you. You can send a request for access, correction, deletion, data transfer of your personal data or a request to withdraw your consent or object to the processing of your personal data to info@optimaalgezondlab.com.

To ensure that the request for access has been made by you, we ask you to send a copy of your identity document with the request. In this copy, black out your passport photo, MRZ (machine readable zone, the strip with numbers at the bottom of the passport), passport number and Citizen Service Number (BSN). This is to protect your privacy. We will respond as soon as possible, but within four weeks, to your request. OPTIMAAL GEZOND LAB would also like to point out that you have the option to file a complaint with the national supervisory authority, the Dutch Data Protection Authority.
